Many thanks for submitting a sample to The Kennel Club Genetics Centre. Please complete each page of this submission form, including Owner, Animal and Sample details. If you have submitted a sample to us before through this process, we will attempt to retrieve previous animals on your account.
On the last page you will be allocated an identifier for your sample; please write this on the swab envelope and return along with any copies of pedigree, health certificates, eye examinations etc that you are able to provide.
